Social Prescribers and Community Partners

Get Active aims to collaborate with healthcare professionals to ensure our programs effectively reach those who need them most. We're open to working directly with Primary Care Networks and similar organisations that frequently engage with inactive individuals.
Potential areas of collaboration include:
Facilitating connections between health systems and physical activity initiatives
Presenting at team meetings to update staff and colleagues, and to develop signposting pathways into relevant programmes
Participating in community events to promote awareness of the Get Active programme
Designing tailored programmes that address the specific needs of individuals or groups, with a focus on key outcomes
If you are interested in collaborating with us, please contact us at activation@leevalleypark.org.uk.
Funding for groups, schools and communities
The Community Access Fund and Active Communities Grant Scheme gives financial support to community and school groups who can benefit from the fabulously diverse and world class opportunities the park offers. Find out more information here.
